RF248-2 - The CORE NECK Tool: A New Approach to Neck Pain for Primary Care

The CORE Neck tool was launched in 2016 by the Centre for Effective Practice in collaboration with the Ontario College of Family Physicians. Dr. Julia Alleyne was a co-Clinical Lead on this project with an expertise in neck pain and rehabilitation. (1)The tool was based on the success of the CORE Back Tool but modified to incorporate the specific needs and evidence regarding neck pain. Neck pain is a common cause of musculoskeletal disability and many practitioners lack confidence in understanding an evidence based approach to assessment and management. (3) This session will address the clinical issues and current guidelines for patient’s experiencing neck pain and highlight the need to differentiate neck pain from headache symptoms. The approach will focus on identifying mechanical etiology while ruling out important red flags such as concussion and cervical myelopathy. The concepts of neck dominant versus arm dominant symptoms will be outlined and high yield questions will be utilized along with a targeted physical examination to differentiate symptoms. Management options will be explored in an evidence based manner which will clearly identify what is recommended and not recommended in the literature for office based care.(6,7) Management strategies will be divided into non-pharmacological options and pharmacological options such as topical and oral medications. Participants will be able to identify the appropriate indications for investigations and referral . (5) Overall, providers will increase their confidence in clinical assessment and management of patients with neck pain. Key messages and resources for patient education will be highlighted.
