Stroke is a crisis event for both stroke survivors and their loved ones. Caregivers are often unprepared to assume the helping role full-time upon return home. As inpatient lengths of stay continue to decrease, there is less time to prepare family for the caregiving role, which has implications for health outcomes, discharge to community and hospital readmission. We will discuss experiences of caregivers, tools to assess needs and supportive intervention strategies. We will also discuss implementation of the Caregiver Early Activation Program aimed at empowering caregiver engagement to ultimately improve rates of discharge to community, hospital readmission and post-stroke complications.
