Prognostic Significance of Diffusion-Weighted Magnetic Resonance Imaging in Hepatocellular Carcinoma Patients after Radiation Therapy

Purpose/Objective(s): To evaluate the prognostic performance of diffusion-weighted magnetic resonance imaging (DW MRI) in hepatocellular carcinoma (HCC) patients treated with radiotherapy.

Materials/Methods: This retrospective study evaluated the data of HCC patients treated with radiotherapy between December 2008 and November 2014 at our institution. Inclusion criteria were patients with DW MRI evaluation before and 1 to 3 months after radiotherapy. The apparent diffusion correlation (ADC) values in all target tumors were measured. In-field control (IFC) was defined as absence of progressive disease based on modified response evaluation criteria in solid tumors (mRECIST). Possible prognostic factors, including alteration of the ADC value after radiotherapy, in relation to IFC were analyzed.

Results: A total of 35 HCC patients with 42 tumors were enrolled. The median follow-up was 10.0 months. The ADC values (mean ± standard deviation) before and after radiotherapy were 1.52 ± 0.34 ( × 10-3 mm2/s) and 1.67 ± 0.31 ( × 10-3 mm2/s) respectively (P=.002). The tumor response was as follows: 4.8% complete response, 69.0% partial response, 16.7% stable disease and 9.5% disease progression. The 2-year IFC rate was 74.8% for all tumors. ADC value increase > 10% after radiotherapy was identified as a predictor of favorable IFC (2-year IFC rate, 91.7% vs 61.6%, P=.027).

Conclusion: ADC value change is related to IFC in HCC patients after radiotherapy. Further prospective studies to validate the prognostic value of ADC change is warranted.
