Proton beam therapy (PBT) continues to be the major focus of a public health policy debate in the United States. Proponents of PBT cite the clear dosimetric benefits of PBT as sufficient evidence to justify its use, while critics point to the high costs and relative lack of comparative effectiveness research. There has been an unprecedented growth in the number of proton beam therapy centers worldwide over the past decade, providing for new opportunities to conduct large-scale clinical trials. There has also been a public mandate for establishing evidenced-based guidelines for use of proton beam therapy.

This session will begin with a discussion ongoing proton beam therapy clinical trials efforts with a focus on the ongoing randomized clinical trials, while also highlighting gaps in the current clinical trial portfolio. We will then discuss clinical trial endpoints that should be evaluated in comparative proton vs. photon clinical trials. This will be followed by a discussion related to ethical issues in proton beam therapy clinical trial enrollment. We will conclude with a case-based discussion highlighting the potential risks and unknowns in proton beam therapy that require evaluation in clinical trials.
