As endodontists, it is easy to assume that any periapical radiolucency associated with pain is of inflammatory origin, especially if the tooth has a significant restorative history. Although a bacterial etiology is by far the most common, it is not a clinical certainty. This case-based discussion will highlight clinical characteristics from the perspective of an oral radiologist, an orofacial pain specialist and an oral pathologist to aid clinicians in differentiating the typical endodontic presentation from that of a potentially more threatening or insidious etiology.
