This symposium will present innovative patient engagement approaches to encourage the much-needed transformation of healthcare from reactive to preventive and person-centered. We will discuss Parkinson’s disease (PD) patient engagement from several rehabilitation research and clinical approaches including PD patients and care partners perception and priorities of their cognitive symptoms. We will discuss the role of patient advocates in the development and implementation of a cognitive rehabilitation and registry for Deep Brain Stimulation in PD. Finally, we describe the development and implementation of a regionalized professional PD network consisting of physical therapists, nurses, neurologists and fitness professionals at community outpatient clinics.
