Description:
Dramatic evolution in the medical and surgical care of children with congenital heart disease (CHD) has led to a growing number of adults with late-onset complications, including heart failure. Growing number of these patients are seen by cardiologists and internists outside of the quaternary centres. Heart failure is an important cause of mortality in adults with congenital heart disease. Recognition and assessment of its severity is challenging even more so in this population.
This session aims at providing key information to the general cardiologist and internist who may care for congenital heart disease patients. Particularities of advanced heart failure evaluation and treatment for these patients will be reviewed.
4 speakers with 15 minutes presentation each followed by 5-7 minutes of discussion and questions or 3x20-25 minutes and a case beginning + return at the end (to see depending on speaker we can find)
Speaker #1: Heart Failure in Congenital Heart Disease: the “rare” are becoming common

-Brief review of the epidemiology of end-stage congenital heart disease
-Overview of the more common complex congenital heart disease leading to heart failure (great vessels transposition, Fallot tetralogy, univentricular physiology…)
Speaker #2: Recognize the failing congenital heart; how to see it coming and acting before it’s too late

-Markers of advanced or deteriorating heart failure (biomarker, clinical event…); what should you monitor and when to call for help
-Unique comorbidities with impact on candidacy for advanced heart failure therapies (liver cirrhosis, PLE, pulmonary hypertension, collaterals…)
Speaker #3: Mechanical support in congenital heart disease; complex connections but promising options

-Congenital heart disease for which MCS have shown good and bad outcomes
-What are the additional challenges for short and long-term MCS in congenital heart disease?
-Important information the surgeon will need to know: shunts, collaterals, previous surgery…
Speaker #4: Psycho-social challenges faced by young adults with congenital heart disease

-Psychosocial comorbidities in the young adult (higher prevalence of depression and cognitive disorder)
-How to engage them in their care and how to identify and address the specific psycho-social barriers and barriers they often face (compliance, school, clinic with “old” patients…)
