Many of the risks health disparities that trans people face can be lowered and even eliminated when people have access to the wellness care and social support that they need. Family Tree Clinic has incorporated a formal community resource department to address the individual needs of each patient. Resources Family Tree provide include, primary care recommendations, Mental health providers, Local gender affirmation surgeons, fertility preservation, Name and gender marker change assistance, Local support groups, notary services and much more. This workshop will examine how Family Tree Clinic has identified the gaps in service and needs that the trans population they serve face and how other clinics and organizations and do the same.

 

Learning Objectives:

• upon completion, participants will be able to Integrate patient resources into a clinic or an organization’s practice.
• upon completion, participants will be able to Identify the specific needs of individual patients/clients and of the local community as well as finding and vetting specific resources.
• upon completion, participants will be able to advocate for systems change when there are no safe and competent services for trans and gender nonconforming people available.
