In May of 2016 , AIDS Resource Center Ohio, a medical center providing care for people living with HIV, expanded our mission. We became Equitas Health: Care for All, providing services including primary care, HIV care, trans health, and more. Our population of patients seeking gender affirming care has grown 300%. Our care model is palliative, based upon patient goals.

 

Learning Objectives:

• 1. Participants will be able to assess readiness for establishment and launch of a gender affirming model of care in a community health center.
• 2. Participants will be able to list strategies for success and pitfalls in the implementation of a gender affirming care model.
• 4. Participants will be able to identify evaluation strategies to support performance improvement, patient experience and disease self-management.
