The majority of pediatric stroke survivors suffer long-term sequelae including motor, communication and neuropsychological impairments (Kirton 2007). Their needs vary by age, stroke subtype and change across the continuum of care. In contrast to developments in the prevention and acute medical management of stroke, rehabilitation research has been limited. The Children's Hospital of Philadelphia Pediatric Stroke Program team will present our interdisciplinary care model designed to support recovery in the child and their families throughout the continuum of care including school and community reintegration. We will highlight challenges and management strategies, and suggest future research directions.
