Over 60% of frail, elderly, and chronically ill patients lose functional capacity during hospitalization. We have developed an assessment tool by refining the extant item response theory (IRT)-modeled Activity Measure for Post Acute Care (AM-PAC) item banks to create a disease-independent multidimensional computerized adaptive test (MCAT). We administered the MCAT to 1600 hospitalized patients and report associations of these data with patient-reported, clinician-reported, objective outcomes, and utilization parameters. We will describe novel methods to integrate response times into trait assessments, include DIF estimates in MCAT parameterization, and estimate the AMC to improve the efficiency of follow up assessments.
