We discuss the use of body-worn acceleromtry as a real-world, objective measure of movement that can be used to evaluate and monitor disability in progressive disease. We show data from controls and children and adults living with multiple sclerosis(MS) or Leukoencephalopathy with Brainstem and Spinal Cord Involvement and Lactate Elevation(LBSL). Multiple sclerosis is used to show that accelerometry can quantify variability in daily activity even with heterogeneous impairments. In LBSL we show that accelerometry is feasible and sensitive for detecting the progression of disease pathology. We will discuss how this information can inform the selection and monitoring of treatment interventions.
