This session will describe the collaboration between a University and a community Free Health Clinic that provides integrated health services for the uninsured and underserved. Care is provided for clients by students in health-related professions (Nursing, Physical Therapy, Dietetics, Respiratory Therapy, Exercise Physiology, Social Work, Medical Assisting and Medicine) with supervision and mentorship by University faculty and licensed health care volunteers. The aspects of care are both related to management of acute problems, as well as prevention and wellness programs, focused to manage chronic disease.

Learning Objectives:

• Upon completion participants will be able to describe the operations of an interdiciplinary comprehensive community free health clinic.
• Upon completion participants will be able to apply this free clinic operational model to their own community.
• Upon completion participants will be able to identify challenges to the implementation of a Free Health Clinic in their community
• Upon completion participants will be able to list interventions for the health and wellness for the management of chronic disease in a community based Free Health Clinic.
