This symposium will cover the spectrum on successful return to community following postacute rehabilitation from the standardized quality measure of successful community discharge to the more holistic and person centered construct of successful community integration. We will first discuss findings from national data examining facility-level variation and correlates of community discharge from skilled nursing facilities and inpatient rehabilitation facilities. We will then discuss common challenges and identify key factors underlying successful community integration among Veterans with TBI.

Learning Objectives:

1. Describe facility-level variation in the new standardized successful community discharge measure for postacute care.
2. Describe both internal and external characteristics associated with successful community integration following postacute care for Veterans with TBI.
3. Describe next steps for optimizing and coordinating care for successful return to community.
