We propose ETAC (Empowered-To-Act-Consistently) communication with four-step method and a scale. ETAC can be used to improve and support patients' adherence. Patients are cued to imagine their future actions in their own environment. Supported evidence in neuroscience will be included to demonstrate the use of imagining. For patients who can participate in a clinical encounter, their intent to adhere to a recommended action can be measured by the ETAC scale. The short ETAC communication engages patients in searching for barriers that will compromise adherence. Through the process, clinicians can gain insight into a patient's perspective to facilitate adherence.
