Katrina Melton , Vice President of Post-Acute Services for Memorial Hermann Health System.
As Vice President of Post-Acute Services, Katrina is responsible for the system strategies and operational models around post-acute services as well as operational oversight for the systems owned post-acute assets as well and aligned post-acute networks. Additionally, she is responsibel for standardizing discharge planning processes and optimizing discharge efficiencies.
Katrina is a Physical Therapist who earned her PT degree at Northeastern University in Boston Massachusetts and has spent the majority of her career working in the post-acute space. She has extensive executive experience both in population health, care management and post-acute hospital operations. Prior to joining Memorial Hermann Health System, Katrina spent the last several years collaborating with large health systems, ACOs and Health Plans to design and implement successful strategies around BPCI, Value Based Contracts and Physician Alignment with a specific emphasis in care management, care transitions and high performing post-acute network development and management.
