The advanced practitioners (APs) who are embedded in the SNF use multiple methods to reduce hospital readmissions. A preferred SNF network and warm hand-offs are used in the transition of care from hospital to SNF. A SNF-ist is devoted to a given SNF from 8:00 am to 5:00 pm weekdays. Designated SNF practitioners provide evening and weekend on-call coverage. This session will focus on how we capture data and use utilize “complicated acute visit” and “skilled visit,” among others, as visit types in the EMR. We'll discuss the work-up and treatment performed by the AP in the SNF and how it prevents patients from going to the ED and/or from being readmitted into the hospital.
